help removing GTV6 starter

Does anyone have page numbers or otherwise some handy information as to how I
can get at the starter motor?  The diagrams I've seen make no mention of how
to remove the darned thing, and I haven't seen one unmounted, so it's quite a
mystery what bolts hold it in and where.
While I'm at it, the passenger side exhaust manifold appears to have a 2 inch
extension of some kind stuck to it.  Do the manifold's studs thread into this
piece as well, or does it even come off?  Again, never having seen a fresh
one, it's kinda hard to tell...
