I have one of those Alfa roof racks on the 105 GTV.  (holding a tobaggan and other toys and junk)  It clamps onto the gutter like most old fashioned roof racks.  I don't know what the Milano needs, but this rack is definitely not exotic except for the Alfa logo and the cradle to hold a bicycle seat and handle bars.
